	LEEP Membership Form For just pennies a day, your membership allows full access to LEEP programs as well as supports individuals with disabilities to participate in life enriching activities. To join LEEP and become a supporter, please complete this LEEP Membership Form and return to LEEP with your annual membership fee.
	LEEP Registration Form A current LEEP Registration Form must be on file at LEEP in order to participate in our numerous programs. This form provides LEEP staff with necessary information such as participants' needs, emergency contact and other vital information.
	Special Olympics Application for Participation Form Special Olympics requires all forms to be filled out in their entirety and on file at LEEP in order to participate in Special Olympics sports. This form must be updated every three years and without the appropriate forms completed, athletes may not participate in Special Olympics.
	LEEP Vacation Registration Form A new LEEP Vacation Registration Form must be completed each time a participants travels with LEEP. This form indicate participants' needs, emergency contact and other vital information. 	Volunteer Application Form The LEEP Volunteer Application Form is required to be on file for any person wishing to volunteer at LEEP. This form provides LEEP with contact information as well as other necessary information regarding the volunteer.
	Volunteer Evaluation Form It is important to receive feedback from our volunteers in regard to their volunteer experience. This information is crucial to improving and maintaining LEEP's current volunteer opportunities.
	Volunteer Time Record The LEEP Volunteer Time Record assists volunteers with keeping records of the amounts of hours they have volunteered as well as informs LEEP as to how many volunteers we receive and the amounts of total hours given.
